Reducing polypharmacy
Polypharmacy describes a time when a single patient is treated with multiple drugs at the same time. Because these medications could be prescribed by multiple providers, the drugs could have overlapping and unnecessary or even counteractive and dangerous effects. Reducing polypharmacy occurs when a medical practitioner safely removes medications from a patient's repertoire until the optimal regimen is prescribed.
