Malnutrition in Clinical Settings: Research Gaps and Opportunities

Goals

The overarching goal of this workshop is to address research gaps and opportunities related to malnutrition in clinical settings. Through research to develop effective nutrition interventions, the hope is to improve health outcomes and quality of life.

Objectives

  • Summarize the state of the science and identify knowledge gaps related to disease related malnutrition and other forms of malnutrition that might interfere with treatment outcomes in clinical settings in the U.S.
  • Suggest innovative research strategies that could help identify individuals that are (or will become) malnourished in a variety of clinical settings and disease states.
  • Examine research to understand, measure, and address malnutrition in clinical settings considering access to health care services and special populations to promote health equity.
  • Suggest opportunities to train the next generation of nutrition researchers addressing malnutrition.
  • Summarize the state of the science and identify knowledge gaps and research opportunities related to clinical malnutrition in the U.S. in pediatrics from infant to teenager, including related maternal/family topics.

Background

An Agency for Healthcare Research and Quality (AHRQ) systematic review on Malnutrition In Hospitalized Adults found that individuals who have malnutrition experience more hospital acquired complications, and have higher mortality rates compared to well-nourished patients. AHRQ defined malnourishment as varying degrees of overnutrition and/or undernutrition, and included participants with many different disease and health states in a clinical setting. AHRQ identified the need for future research that addresses the effectiveness of various measurement tools for malnutrition on clinical outcomes and hospital-initiated malnutrition interventions to decrease mortality and improve quality of life. This workshop relates to Strategic Goal 4 of the Strategic Plan for NIH Nutrition Research, to reduce the burden of disease in clinical settings, and improve the use of food as medicine.

We are grateful to the American Society for Nutrition (ASN)’s Medical Nutrition Council, the American Society of Parenteral and Enteral Nutrition (ASPEN), and the Academy of Nutrition and Dietetics (AND) for their assistance in planning the workshop.

This workshop is led by the NIH Office of Nutrition Research (ONR) Medical Nutrition Implementation Working Group. This working group is charged with implementing the Strategic Plan for NIH Nutrition Research. It focuses on research aimed at reducing the burden of disease in clinical settings and how to improve the use of “Food as Medicine.” The ultimate goal is to enhance clinicians’ and the public’s ability to access evidence-based “medical foods,” medical nutrition therapy, and specific eating patterns. These may help prevent or reverse chronic diseases or to improve the effectiveness of medical treatments.

The working group is collaborating with the American Society for Nutrition (ASN)’s Medical Nutrition Council, the American Society of Parenteral and Enteral Nutrition (ASPEN), and the Academy of Nutrition and Dietetics (AND) on the development of this workshop.
