Connected and Coordinated Care Management

The Optum Clinical Management (OCM) team chose to think differently as they considered how a provider would deliver distinctive Care Management to our members. Clinical leaders with decades of experience including running an ACO to being a trauma nurse in the emergency room worked in coordination with technology leaders to develop an innovative and personalized Care Management Product. In this session, you will hear from some of our clinical leaders, their personal stories and how those experiences have shaped the Care Management Product. You will be the first people to see this new product and how we were able to deliver a team-based, patient centric approach to managing medical conditions effectively. You will get a preview of how technology will help clinicians to coordinate with providers and personal care givers to provide care for our patients bringing empathy and compassion back into care management.
